Division of Insurance applicants must bring the required authorization form and confirmation information.
- Government-issued photo ID such as driver license, passport, or ID card
- Nevada Division of Insurance Fingerprint Authorization Form
- Sircon Confirmation ID or NIPR Transaction number on the form
- Payment method, cash or card
Applicants without the required Nevada Division of Insurance paperwork cannot be fingerprinted.
Required Paperwork Before Fingerprinting
Applicants must bring official paperwork from the requesting board, employer, or agency. A code by itself may not be enough because the office may need to sign, stamp, and provide a TCN number after fingerprinting.
